GROUP COACHING AGREEMENT State of Arizona By enrolling in any group coaching program offered by Heather Lena, LLC (“Company”), the participant (“Client”) agrees to the following terms: 1. Nature of Services Group coaching is a personal development service. It is not medical care, psychotherapy, psychological counseling, or legal or financial advice. Client is solely responsible for their physical, mental, and emotional well-being and agrees to seek licensed professional care where necessary. 2. Assumption of Risk & Release Client voluntarily participates and assumes full responsibility for all decisions and outcomes resulting from participation. To the fullest extent permitted under Arizona law, Client releases and holds harmless Heather Lena, LLC, its owner, agents, and affiliates from any claims, liabilities, damages, or losses arising from participation. 3. Payment & No Refund Policy All sales are final. No refunds, exchanges, credits, or transfers are permitted. Missed sessions are non-refundable. Payment plans must be completed regardless of attendance. Chargebacks constitute breach of this Agreement. 4. Session Structure & Recordings Sessions are conducted virtually unless otherwise stated. Sessions may be recorded. Paid participants will receive limited access to recordings for personal use only. Recordings may not be downloaded, shared, or distributed. The Company reserves the right to adjust scheduling as necessary. 5. Confidentiality Client agrees to maintain confidentiality regarding other participants. Confidentiality within a group environment cannot be guaranteed. Disruptive behavior may result in removal without refund. 6. Between-Session Messaging If included in the purchased package, limited messaging support via Voxer or designated platform may be provided. Messaging is for brief clarification or accountability only. Responses will occur within 24–48 business hours during normal business hours. Messaging is not unlimited coaching, crisis support, therapy, or emergency care. Client agrees to contact appropriate emergency services or licensed professionals for urgent matters. Company reserves the right to limit or discontinue messaging if boundaries are exceeded. 7. Intellectual Property All materials and recordings are the exclusive property of Heather Lena, LLC and may not be reproduced or distributed. 8. Governing Law This Agreement is governed by the laws of the State of Arizona. Venue shall be Navajo County, Arizona.
